Study Description:
This study is testing an investigational oral medication for patients with heart failure and impaired kidney function. Participants will take either the investigational medication with an FDA approved medication, or the FDA approved medication alone, daily for on average approximately 2 years. You will be asked to attend a screening visit which may include an ultrasound of your heart if you have not received one within the last year. Total participation will last approximately 3 years and would include attending several doctor’s visits scheduled approximately 1-4 months apart. Eligibility for the study will be determined by the study doctor following in-person screening visits to the clinic. You may be compensated upon the completion of all study procedures.

Qualifications:
- Within the last 6 months, must have been hospitalized or had an urgent visit for worsening symptoms of heart failure
- Must currently be on medication to treat your heart failure
- Cannot have type I diabetes
- Cannot have had heart attack, stroke, mini stroke, heart valve repair or replacement, or pacemaker placement in last 3 months
